1. B. The Plan Do Check and Act cycle (PDCA) starts with a plan to change, test or improve. It is followed by implementation of the plan or do. Once complete the activity or process is reviewed and evaluated which is the check phase. Based upon the outcome, a change or recommendation is made called an act.
2. A. Outcomes monitoring by definition is the measurement of structure and process. Examples are clinical progress, infection rate, bed access, and so forth.
3. D. Improvement initiatives
need to be linked to the organization’s core values because of the relationship
and description of what is important and culturally acceptable. Linking to the
mission is imperative because initiatives need to be consistent with the organization’s
commitment. Finally, the vision defines the direction and improvements needed
